Dr. Chilvers offers appointments to Lambton County patients who have a completed referral form from their family doctor or nurse practitioner. (Until September 30, 2022, all appointments will be virtual).
Request your family physician or nurse practitioner to fax a referral to 855-943-3262.
Once your referral has been faxed to Dr. Chilvers you will be contacted with an appointment date and intake form.
